jackrabbit: In the storyline, the cocks on the x-girls are not permanent. The ability to morph the genital area came about from an accident involving Rogue, Mystique, and the sports drink Pow-R 8, which has disastrous effects on mutants. So Kitty doesn't have a cock all the time. And presumably not when with Lance. In this scene, Jean mentally forces Kitty to morph against her will.
