BenedictWolfe: They release a "tadpole" (which I guess means they have a uterus like reproductive organ) into the communal brain pool. The tadpole matures and is then stuck into the ear of a human. It digs in, eats the brain and morphs the body into an illithid. SEXY!

Anonymous5: Yep :-). I'm the one who commissioned this pic. In my 'story', when the tadpole that made this illithid transformed the human, a 'remnant' remained (the penis ^_^). This illithid decided to make the most of its singular, unique appendage :-)
